OEM # F3LP7A040AA (note this is the same part number stamped ont he 11-1/4'' tail housing as well.  

 

This extension housing has the hole with a plug to be used either way.

    A couple areas were broken. Thankfully we were able to find a way to fix it and get our vehicle running. Still think it's a good company to use.
